Insurance

Anthem Blue Cross/Blue Shield, Bluegrass Family Health, United Healthcare, CHA Health, Humana, Medicare, Worker's Compensation, Automobile Insurance, Cigna, Aetna; this list is not all-inclusive, but lists the primary carriers we accept.  If we are not a participating provider with a particular company or policy, we will make the effort to become one.

  • Cornerstone accepts most insurances including Commercial plans, Medicare, Workers' Compensation, and Automobile insurance.
  • Most insurances do provide benefits for outpatient therapy, however, these benefits are different for each policy.
  • Cornerstone will file your insurance claims for you.
  • Cornerstone will assist you in determining your insurance benefits, however, it is ultimately your responsibility to obtain this information.
  • We highly recommend that you contact your insurance carrier and learn what your benefits are.  There should be an "800" number on your insurance card that you can call to obtain this information.
  • Your insurance policy may require you to pay a deductible, a co-pay, and/ or a percentage of the costs of your therapy treatment.
